Before beginning operation of the truck, check each item below. Notify the
supervisor of any items that require maintenance and remove the truck from
service until repairs complete.
Ignore any item that does not apply to the truck being inspected. Explain problems
noted below or describe additional problems below the chart.

Overhead Guard: damaged, loose or missing



Hydraulic Cylinders: damaged, leaking



Mast/Carriage Assembly: damaged, loose or missing parts,
excessive wear, leaks



Lift Chains and Roller: damaged, uneven chain tension.



Forks: damaged, bent, worn.



Tires and Wheels: damaged, worn, loose.



LP Gas Tank and Hose ((LP) gas trucks only): tank secure,
hose damage, leaks, gas odor.



Seat and Seat Belts: damaged, loose or missing?



Warning Decals/Operators Manual: missing and not
readable?



Data Plate/Load Chart: missing or not readable?



Battery: properly installed, caps and cables tight and
undamaged, charged, proper fluid level.



Gauges and Instruments: damaged or missing.



Engine Oil: check level, dirty, leaks.



Engine Coolant: check level, leaks.



Transmission Fluid: check level, leaks.



Hydraulic Fluid: check level, leaks.

Mast: lifts and lowers smoothly, chains and rollers operate
properly, excess noise, fluid leaks.



Carriage and Attachments: tilts forward and backward

Steering: works properly, excess free play, leaks.



Horn: does it works properly?



Control levers: operate properly, loose or binding, returns to
center freely.



Directional/Speed Control: operates properly, loose or
binding, returns to neutral freely.



Drive Axle: noise, lubricant leaks



Service Brakes: does truck stop properly in forward and
reverse?



Parking Brake: can the truck move with the brake applied?



Back-up Alarm (if equipped): does it work?



Battery Check (electric trucks): does battery show full charge
under load?



Safety Door and Switch (if equipped): in place and working
properly?



Work Platform (if equipped): does platform raise and lower
smoothly?



Additional Problems:
