SFC Program in Georgia Offers Financial Support &
Coaching to Family Caregivers
Funded through Medicaid, the Structured
Family Caregiving (SFC) program provides
financial support and health coaching to
unpaid primary caregivers.
The SFC is available
through care agencies, such
as Home Nurse Inc, which
has been a trusted partner
since 2019.
The Griffin-based
Home Nurse Inc
is currently
accepting
applications for
the SFC.
To be eligible for the SFC
program, you should be
the primary caregiver for
an elderly and/or disabled
loved one; you cannot
have employment or run a
home business.
You must live in the same
home as the person you care
for, and be related to them
either biologically or through
marriage; spouses and legal
guardians are not eligible.
Lastly, the person
receiving the care must
be Georgia Medicaid-
eligible and a participant
under the CCSP or
SOURCE waiver
program.
